2. PATIENT CASE #2S: No change in gait instability. When the patient had to lie quietly with neck extended, gait instability was much worse for 20 to 30 minutes after the test. Medications: Warfarin, digoxin, verapamil. O: Alert. Ataxic gait with foot slapping and instability in tandem walking. Mild distal weakness and wasting. Barely detectable DTRs. Impaired vibratory sense below the hips. Impaired position sense in toes. Head CT shows diffuse atrophic changes. EMG: Distal demyelinating axonal neuropathy. A: Gait disorder with central/peripheral components in the context of cervical spondylosis and peripheral neuropathy. P: Have patient obtain a B12/folate test. Reassess in one month.
