INSTRUCTIONS The purpose of this task is to provide documentation related to aspects of case management in the workplace. Your portfolio can be set up using a display book, A4 binder with plastic pockets or A4 notebook. It should be clearly detailed and labelled according to the evidence required in this task. You are required to provide copies of the following documents: Workplace case management plan template Workplace policy and procedure on client privacy and condentiality A position description of your role Documentation process and protocols related to case management Meeting agenda - from one case management meeting you were involved with in the workplace DDDDDD Workplace procedure on the exit or case closure process. You will need to be able to prove that you have created each document. You must get your supervisor to sign each document that you add to your portfolio. This will also provide evidence that you have permission to remove the document from your workplace
