The program should ask the user 4 questions:

Name, Address, City, Zip code

Processing:

Store the name, address and city in three separate string variables

Store the zip code in an integer variable

Output:

Print the header "Your Mailing Label is:"

Print the name on one line

Print the address on the next line

Print the city, state, and zip code on the third line
