Lab 2: HTML Form

Goal of this lab: Understand and practice HTML forms.

You will create a web page that has a form. The topic will be about Social Media Usage

Your web page must include following elements:

• A Heading on the top Social Media Usage Survey.

• A paragraph that describe the survey purpose.

Example, We created the Social Media Usage Survey to get a better understanding of your employee, customer & target audience behaviors based on habits, interests, level of participation and current involvement across a variety of social media channels.

• A form that has:

First take the user information: Name and Age

Second, at least 5 questions applying different question input types to collect the data

• Radio button
• Checkbox

Third, A submit button, and a Rest button as well.

• Note that, you can use any tags we learned before such as:

1. How many social media sites do you have accounts with?

2. How many times a day do you look at social media?

3. How much time do you spend on social media per day?

4. How often do you post on social media?

5. When do you access social media?

8. What do you use social media for?

9. Has social media effected any of your relationship?

10. Do you consider yourself addicted to social media?
