Make a program-level logic model outline in which you consider inputs, activities, outputs, and outcomes. The program level is to transition a clinical and admissions team for an addictions facility into the same building. As the transition is taking place current client's will not be able to be seen and will have to face being separated and not treated. Also once staff is in the same building it will be an adjustment to work together.
