A 27-year-old female living i;n Oxford, MS. Born in Denver, Colorado. Received her bachelor's degree in chemistry at the U1niversity of Denver. Moved to Oxford 2 ye=ars ago to work as a Chemist/ry Lab Assistant.

Close to her m[other. Father died from liver cancer when she was 16 years old. She used to be very close to her sister who di/;ed a year ago in a car acciden.;t in whic

h the patient was a passenger.

She indicat;ed to have no history of prior psychiatric evalu;ation or diagnoses.

She denied any past or current suicidal and/or

homicidal thoughts, ideation, plan, or attempt.

History of problem:

The patient reports feeling fearful and uncomfortable insocial situations since the age of 14. These feelings have become worse when her sister d;ied. Her sister usually helped her become more socially engaged and was the main reason for her moving to O/x>ford

She describes havingpanic attacks (i.1e., suddenly feeling anxious, frightened, and uncomfortable) when near people but also without apparent causde since the age of 22. She believes thes;e panic

attacks to be connected to her sister moving away from home a ound the same time.

Since her sister's death she reports experiencingavoidant behaviors related to her sister's death, trouble recalling d2etails of the accident, feeling de\tached from others, and feelin|g numb. Additionally, she has been sleeping worse and has been e.asily startled. The patient re\

fuses to talk about the accident or her sister's passin1g in detail.

Her dis;comfort /and fear in social environmefnts have become pe|rvasive enough for he-r to avoid all social situations apa"rt from the occasional communication with her boss./
