The receptionist at the hospital insisted that admission forms be completed before Hardy could be sent to the emergency room. Sam was concerned that Hardy needed urgent attention and pleaded that the receptionist admit Hardy while he completed the forms. The receptionist agreed. Sam completed as much of the form as he could & handed the forms to the receptionist. She told him that he had to sign it, which he did. He noticed that the form had about a half a page of printed text above the signature line, but he did not read it. Had he done so, he would have realized that he had signed a standard form, but with a very unusual provision that obligated the signatory to pay all the hospital charges, if the patient could not.

Sadly, Hardy died. When it was discovered that he had no money to pay the bill, the hospital demanded payment from Sam, based on his signature on the form.
