One role of a supervisor or manager is performance review. Most companies have an annual review of their staff and at this time consider performance and salary increases.

Your assignment is to develop a performance review - this is a document that would be given to your employee to rate their own performance and also for you as a supervisor to rate the employee. I ask that you put this together keeping in mind it is not for anyone specific - this is a form that would be given annually in an organization. The assignment will be worth 30 points.

You can use a number rating system, yes or no, agree / disagree, etc.
