After obtaining necessary permissions from the Vice-Chancellor for Health Affairs of Shiraz University of Medical Sciences and also from the heads of three health centers, to protect participants belonging to the same community from study information contamination, the authors took one urban health center as a cluster in the randomization. There are eight government diabetes referral clinics in Shiraz, out of which, three centers were selected by simple random sampling method and then were allocated to a control and two intervention groups through simple randomization. The statistician was blinded to group allocation in this study
