Part 3B: Development of Case Plan

Based on the information received during the role-play you must create a case plan for Justine that reflects the choices made during the assessment process and role-play. The information needed to complete the plan will include:

1. Any referrals made and how strategies, services and resources will be implemented to facilitate recovery.

2. The plan will include a follow-up and evaluation section to ensure records are maintained and evaluated accordingly. (Appendix 1)
