When we consider patient-centered care as one of the drivers of achieving high-quality care, we also must consider the social determinants of health that have a great impact on access to care and health literacy. This article discusses the factors that contribute to the dysfunctions in our current healthcare system. What would you recommend to improve a team-based approach to health care and what innovative technologies are needed to support the improvement of care across different patient groups?
