You are required to analyse each person's needs. This involves:
- Working within the scope of your own role as relevant
- Working with the person to identify and prioritise their needso Evaluating needs and complex issueso Assessing potential risk factors for service delivery
- Determining appropriate services and providing information to each person about the services available.
In Section 1 you will describe how you analysed each person's needs, as well as determined suitable services.
You are required to fill in answers to all of the questions in Section 2 and have your workplace supervisor endorse your evidence.
You must complete section 2 of the portfolio for each of the people you work with.
