The lesson plan is divided into three tasks. For the first task describe the use of reading assessment data applicable to the lesson.Prior to developing your plan, review the model lesson plans found in Chapter 8. They are solid examples of the level of detail needed to meet mastery criteria.

2. Identify the screening/diagnostic assessment used to assess fluency and comprehension.

3. Identify the fluency and comprehension need uncovered by the screening/diagnostic assessment for this group of regular classroom students which might contain a student with an IEP or 504 plan.

4. Provide any additional comments (Optional). Identify any information concerning the target student (student with an IEP or 504 Plan) that you think your course facilitator may need to know to understand your differentiated instruction lesson better.

5. Considering the assessment data and the fluency and comprehension need identified above, state the goal for this instruction.The focus of the goal must be on improving an identified need.
