Question 5 Data Table Design 1 (20 points)

You are working for a healthcare clinic and you are asked to develop a database that captures the data about physicians, patients and visits. Design the tables PHYSICIAN, PATIENT, VISIT. You dont need to enter data in the tables.

a) Each table has to contain at least relevant 5 fields. Identify the field name and relevant data type (e.g. numerical, text, alphanumerical, date, currency, yes/no) for each field. (0.5 point for each field name in each table + 0.5 point for each data type each table = total 1 * 5 * 3 (15 points)

b) Mark the primary key for each table. (1 point for each table, total 3 points)

c) Mark the foreign keys on the relevant table. (1 point for each foreign key, total 2 points)
