Create the survey questions using a Likert Scale (strongly agree, agree etc...) covering the following elements.

Ease of use	information on earnings, costs and variances clear and meaningful
Accounting systems	Employees knowing how to use financial packages
Training	Employees given enough training on systems, processes and information
Accuracy	The financial processes cover all costs, earnings and relevant rations, variances
KPIs	These need to be relevant to what is being measured e.g. SMART
Communication	Is communication clear and two-way?
Procedures	Are there clear and effective procedures to follow?
