Elizabeth "Liz" Smith is a 31 year-old single, Caucasian heterosexual cis-gendered female. She is currently working as an accountant at a successful firm in an urban city. Liz has been employed with his company for several years and has always been a high achiever. She graduated with honors in college and is a top performer in the company. In the past she has had very high standards for herself and can be very self-critical when she fails to meet them. In college she was very studious and was described by her friends as quiet and shy. She didn't date very much and she didn't have a long-term committed relationship until after she graduated college. She started dating someone that she met online about 3 years ago. During the relationship, he cheated on her several times, but she stayed with him anyway. She eventually broke up with him about 3 months ago and has experienced feelings of sadness since.
Lately, she has experienced feelings of worthlessness and shame due to her boyfriend's infidelity and recent status as a single woman. Her coworkers have noticed that she is often irritable and withdrawn, which is quite different from her typically upbeat and friendly disposition. Recently, she has called in sick on multiple occasions which is completely unlike her. On those days, she stays in bed all day, watching TV or sleeping.
She has also increasingly isolated herself from friends and family who have noticed changes in her behavior as well. She has been tearful and frequently cries on the phone when talking with friends. For the past few months Liz has felt dissatisfied with her life. She gets frustrated with herself because she thinks she should be happy and grateful for her job and family. Upon the recommendation of a friend, she decided to seek out counseling.
