I will be interviewing a 47-year-old African American single, heterosexual male by the name of Albert Roland. He is being assed for ongoing issued with his mental health and substance abuse. However, my client has been having problems with all these issues which has cause him to lose his job, relationship issues with children, low self-esteem, anxiety, depression, and suicidal ideation. He has been struggling with his systems for over 11 years.

Primary presenting problem:Albert is struggling with his substance abuse and meanwhile trying to establish a social life.

Long term goal:Albert will focus on stop drinking overall.

Objectives: Albert will reduce his intake consumption from 7 days to 4 days a week.

  1. Albert will utilize a weekly schedule/log to monitor his alcohol intake. Albert will set a reminder in his phone which will help him prioritize his responsibilities on a daily basis for the next two weeks, as evidenced by successfully recording his intake completion of his daily log. SMART-Albert will plan and use the weekly log daily provided for him at least every 2 weeks to complete his assigned tasks with 100% accuracy as evidenced by the case manager.
  2. Albert will manage his time effectively and prioritize his substance abuse daily for the next two weeks, as evidenced by successfully completing his daily tasks and weekly log. Albert will complete 1-2 meetings every week and report to his case manager in two weeks. SMART-Albert will complete his weekly log in allotted time frames with a minimum of 100% accuracy, as evidenced by the case manager feedback for the next 2 weeks.

Develop a plan for evaluation and termination with that client. This plan must be professionally organized in the third person and address the following in order:

  1. Evaluation
    1. State the SMART goal that was identified by this client.
    2. What will you need to measure in order to monitor this client's progress and what baseline data will you need to gather first?
    3. From the list of evaluation strategies Baseline Data, Evaluation, and Achieved Goals. Select the most appropriate one to utilize with this client. State the reason for your choice.
    4. Discuss how you plan to implement this strategy, including frequency.
    5. How will you and your client know when they have met their goals and are ready to proceed with termination?
  2. Termination
    1. In your final sessions with this client, how would you provide them with a sense of closure? Be specific.
    2. What factors would you need to consider when discharge planning for this client in order to ensure that the client is supported after termination and able to maintain progress? Be sure to identify allpotential referrals.
