Use the coding function of the Health Records Department to create indicators that would fall into the 4 Quality Dimensions of Timeliness and punctuality, Accuracy and reliability, Accessibility and clarity and Relevance. In other words, think of indicators that a coding manager might use to monitor his or her staff, or that the administration might use to monitor the coding department. Read each definition carefully. Example: A 'Timeliness and punctuality' indicator for the chart completion process: # of charts completed within 14 days of discharge. This would be used by supervisory staff to monitor the timeliness of physician compliance with the incomplete chart policy and would trigger a reminder to physicians with chart deficiencies. A low # might suggest potential issues to be investigated including documentation, 'bottle necks' in record processing, etc. Typically, the department would have a target they want to reach, such as "90% completed".
