SECTION A COMPULSORY QUESTION (15 marks) QUESTION ONE (15 marks) CASE SCENARIO The pharmacy at a hospital fills medical prescriptions for all patients and distributes these medications to the nurse stations responsible for the patients care. Medical prescriptions are written by doctors and sent to the pharmacy. A pharmacy technician reviews the prescriptions and sends them to the appropriate pharmacy station. Prescriptions for drugs that must be formulated (made onsite) are sent to the lab station, prescriptions for off-the-shelf drugs are sent to the shelving station, and prescriptions for narcotics are sent to the secure station. At each station, a pharmacist reviews the prescription, checks the patients file to determine the appropriateness of the prescription, and fills the prescription if he/she is satisfied that the dosage is at a safe level and it will not negatively interact with the other medications or allergies indicated in the patients file. If the pharmacist does not fill the prescription, the prescription is returned to the prescribing doctor and the doctor is contacted to discuss the situation. The prescription may ultimately be filled or the doctor may write another prescription depending on the outcome of the discussion. For each prescription that is filled, the pharmacist will access the particular patients record and create a record in the prescriptions file with an auto-generated unique identification number, date, the drug prescribed, quantity dispensed, expiration date and the dosage instructions. The Billing Department accesses the pharmacy system for data from the prescription file and the particular patients record to generate an invoice for the system which the pharmacy will eventually send to the patient. A prescription label is also generated listing the patients name, the drug type and dosage, expiration date and any special instructions. The label is placed on the drug container and the prescription and the container are sent to the appropriate nurse stations. a) Draw a Context Diagram of the prescription distribution system at the hospital from the perspective of the pharmacy. (7 marks) b) State the main sub-processes mentioned or inferred from the case for a High Level Diagram of the prescription distribution system at the hospital from the perspective of the pharmacy. (4 marks) c) For each data flow on the Context Diagram show the appropriate sub-process mentioned above to which it will be attached. (4 marks) SECTION B DO THREE QUESTIONS (45 marks) QUESTION TWO (15 marks) a) Draw the initial full Decision Table to represent the following case. A client who has all monthly premium payments up-to-date will qualify for 100% reimbursement of the amount claimed, provided that the client visited a doctor who is a preferred provider and visited the doctor at the doctors office. If the client has all monthly premium payments up-to-date, the client will qualify for 75% reimbursement of the amount claimed, provided that the client visited a doctor who is a preferred provider or visited the doctor at the doctors office. If the client does not have all monthly premium payments up-to-date, but visited a doctor who is a preferred provider or visited the doctor at the doctors office, or both, he is given a 50% reimbursement. In all other cases, no reimbursement is given. (6 marks) b) Draw the Reduced Decision Table to represent the combined rules of the case given in (a) above.
