The additional instructions you need for Assignment 2 are as follows: Belinda has decided to act on your advice and instructs you to commence court action on her behalf. She provides you with the following information to complete her claim: The medical report provided by Belinda's treating doctor at the hospital describes her injuries as: 1. Displaced fracture affecting Tibia Bone 2. Fracture to humerus bone 3. Second degree burn on cheek Belinda was in immense pain when lying injured in the hotel room and was terrified no one would come to help her. Belinda had to pay for her ambulance to the hospital - this cost her $700. She had to have surgery to her ankle, and the break was so bad they had to insert metal pins during the surgery. This surgery cost her $4000. She also broke her arm and that required a smaller surgery for realignment of the bone and then her arm was placed in a cast for a duration of 4 weeks. The surgery and casting of her arm cost $2500. She had to have treatment to her burn including regular dressings and monitoring so it did not get infected. This cost her $1500. She needed to stay in hospital for four weeks. The cost of her hospital stay was $7000. Upon release from the hospital, Belinda has to attend physiotherapy twice a week for four weeks and then once a week for eight weeks. Each physiotherapy session costs $70. The taxi to and from each session costs her $15. Belinda has to take medication to manage her pain as her ankle and arm heal. This costs her $40 per week for twelve weeks. Belinda has not been able to attend work. She used up her two weeks sick leave and her doctors have said it will be at least six months until she has enough strength in her ankle to return to her cleaning job. She has been put on unpaid leave for this time, meaning she is not getting her monthly income of $2500 plus her Pacifica National Provident Fund superannuation payments of 8% of her income She has to walk on crutches until her ankle is fully healed. She used to be very active and enjoyed joining a weekly social walking group. She cannot do any extensive walking while her ankle is healing and so she has lost her social network. Belinda has also attended four counselling sessions because she is feeling very depressed and anxious after the incident. Each counselling session is $60. Belinda regularly has nightmares about lying in the hotel room in pain waiting for someone to come and help her. Belinda's husband has noticed how depressed she is and that she mostly stays in bed with the curtains drawn. She only gets up when he brings her food, when friends visit or when she needs to attend medical appointments. Prepare the following documents: 1.
