Subject: Interoperability healthcare personal mobile apps where older adult can have access to all their personal healthcare record in one place to communicate with other healthcare professionals anywhere

The Report should proposes the implementation of a health information technology/system (IT/IS) or digital health solution in a long-term care home or community healthcare settings. The proposed health technology system/solution should address one of the many challenges observed in this environment and in relation to the continuity of care for older adults.

The ministry of a long term care in Ontario is accepting proposals for the implementations of IT solutions in long-term homes that can support the delivery of care and services provided to residents.
