The Report should proposes the implementation of a health information technology/system (IT/IS) or digital health solution in a long-term care home or community healthcare settings. The proposed health technology system/solution should address one of the many challenges observed in this environment and in relation to the continuity of care for older adults.

The ministry of a long term care in Ontario is accepting proposals for the implementation of IT solutions in long-term homes that can support the delivery of care and services provided to residents.
