	1)ALL team members should be listedon your reporting sheet,including yourself.
	2) Thenames should be in alphabetical order, by first name.
	3) The scores in the first three (2) columns are scored on a 1 to 5 basis, with3 being the average gradeThat means that 75% of the scores in those three columns should be "3".
	4) The score in the "Overall Contribution" column should reflect the percent of contribution to the overall effort that specific team member contributed to the work the team did that week.When the percentages in the "Overall Contribution" column are added up, all of the numbers in that column should total 100%.
	5) If any of the scores in the "Overall Contribution" column are10% or less, you should give a brief explanationas to why you gave the score you did.That will help the instructor know the best action that needs to be taken to help the low scoring team members as well as to help even out the work contribution of all of the team members.
