Create a Job on E-Quip with a Job type of Provide EBME Loan and Status of In Progress and include the following details: Technician field Name of the engineer who will be responsible for delivering equipment to requestor (and who will then be responsible for monitoring the loan period and recovering the equipment back) Reported Fault Engineers initials, who requested the loan equipment, contact number, its location and any dates/comments when the customer is contacted. Work Done Original equipment repair job number which the equipment is loaned against, what loan accessories and if a User Manual was supplied 4. The requestor should sign the Job Request form F180 whenever possible, when the loan equipment is handed over. If it is collected from Medical Engineering, the person collecting it should sign. 5. When deemed necessary the engineer delivering the loan equipment shall complete a Loan Agreement form QAR076 defining the job number and loan period. The senior department user shall complete the user section of the agreement form, including their signature authorising payment for the non-return of the loan equipment including loan accessories when applicable.6. File the outstanding Job Request Form F180 and Loan Agreement form QAR076 where applicable in the Equipment on Loan file for the duration of the loan. 7. The Loan equipment including loan accessories should be regularly monitored by the allocated engineer by means of the Engineer in Progress Jobs. The allocated engineer should recover the loaned equipment once it is no longer required. The user should clean the loan equipment and attach a Trust green decontamination label to comply with Trust Decontamination Policy before it enters the Medical Engineering Department. 8. Loan equipment including loan accessories (and user manual if supplied) returned / collected from the user, should be placed in the Booking-in area. The outstanding Job Request form and Loan Agreement form QAR076 should be removed from the loan file by the person booking in the job and update the job completed status on E-Quip. The Job Request form and Loan Agreement form QAR076 where applicable should be placed in the completed jobs file.
