"The story of Judit and Badong"

"Judit" is a PT who contracted with the school system to provide physical therapy services to a public high school in a small town. Her duties include preseason screening of student athletes and physical therapy treatment of injuries acquired during training sessions and games. "Badong" is a 12th grade student and the school's representative to the regional wrestling and weight-lifting championships.

The team's coach noticed that Badong appeared to be experiencing weakness in his left arm. The coach directed Badong to be examined by the team's physician who referred Badong to the physical therapist for evaluation and treatment. After completing her evaluation, Judit informed Badong that he had a minor tear in his tendon at the point of biceps brachii muscle insertion. As the tendon tear began to heal, Judit noted an excessive amount of muscle hypertrophy. She asked Badong whether he was using any steroids. After several evasive answers, Badong admitted using contraband methandrostenolone. Judit knew that methandrostenolone was an anabolic steroid available only by prescription and used in the treatment of osteoporosis. She also knew that illegal doses of the drug were readily available in weight training rooms and sports centers for athletes who believed that the drug could increase muscle bulk.

Badong begged and threatened Judit to keep his secret. He said that he would take the drug only until the regional championships, four weeks away, and then he would stop. He told her that all of the athletes used steroids and that he needed the drug to be competitive. She informed him that side effects of the drug included testicular atrophy, liver disease, and cancer. He answered that he did not care whether he died at 30 years of age, that winning the regional championship was the primary goal of his life.

Judit pondered the situation. She reflected that the team's coach was known for his aggressiveness and winning record. She knew that he chose to ignore drug abuse among his athletes, especially if they were winning. He would dismiss them from the squad the following season if they could not match or surpass their performance records. Judit also knew that Badong was under a considerable amount of pressure. The entire town was talking about Badong's chances in the championship. Badong had been the focus of an article in the hometown newspaper, the feature subject on a local television station's sports report, and the star of a school-wide pep rally.
