The student nurse reports to you that Cora fell during the day at approximately 0800 and Cora did not ask for help when she needs to get up but seems to be fine this morning. The student nurse states that Cora fell as she was getting up to use the commode and Cora denies loss of consciousness. Cora has a 4 cm abrasion on her leg. You instruct the nurse to monitor pain and mobility issues. You tell the student nurse that you will be making a home visit this morning for an assessment of the client's condition and situation. You visit the home that morning at 1020. You observe a 3 cm abrasion on the left shin but she has no complaints of pain at this time. BP 170/100, T 38.3 C (t), P104, R24. Cora's vital signs slightly increase due to the fall and maybe, there is an infection in the wound. You apply a clean dressing to the wound and obtain a wound culture specimen. Her daughter states that the fall was not witnessed, but she assisted her mother after the fall because the nurse was not there. Also, Cora tells you that she called the student nurse many times before she attempted to get up, but with no replied. So she tried to get up by herself and use the bedside commode when she got dizzy and fell. Cora also expresses that she feels scared to get out the bed now. You were surprised that the student nurse did not tell you this. You called Cora's physician, Dr. Yu and he was notified of her fall. New orders were received for analgesic for pain.
