The underlying problem with the Waterfall Model of development is the requirement that everything is finalized and signed off as done before moving to the next phase results in wasted effort trying to address all eventualities and defects moving forward with no way to back up and correct the root cause or the consequences of the defect if the work has already been signed off as "done".
