Case Study Background

It is two (2) months post the implementation date and you have received reports from the Department Managers on the implementation of the Fatigue Management System (FMS) including the following documents:

  • Implementation Plan; and
  • Fatigue Management Policy & Procedure

The contents of these reports include the following:

  1. Departmental meetings with staff when well
  2. All staff have signed acknowledgement of the FMS documentation
  3. All managers have undertaken Fatigue Management Training
  4. All Manager found it to be helpful
  5. One (1) Manager has identified that the Fatigue Management Policy & Procedure has not been correctly implemented. It was reported that a delivery truck driver was forced, by the sales team to drive 16 hours straight without a break to meet a promised delivery deadline after a customer insisted on delivery to a rural remote location by the next day.

Outline what an appropriate managerial response would be to item (5) in the reports and outline a related action you could implement to ensure this don't happen again.

Case Study Background Information In light of the issued identified in questions 6 &7, you have been asked to hold appropriate discussions with relevant personnel to ensure NCT aperational systems and operating procedures are compliant with the fatigue management requlations and policy. At the meeting, there is six (6) attendees including yourself. The meeting attendees are as following: 1) Joe Crane (the Truck driver) 2) David Conrad (Depot Manager) 3) Ivan Harvey (External WHS Representative) 4) Heather lones (HR Manager) 5) Dale Batterson (Sales Manager) 6) You (Consultant) David chaired the meeting. David opened the meeting by asking the attendees for feedback on the Implementation Plan and the Fatigue Management policy and procedure Ivan expressed concerns with NCT compliance with its own policy. It has clearly not been following, and sales putting unrealistic delivery targets on drivers to meet their monthly sales targets where not safe business practices. Heather agreed and requested NCT systems be audited to evaluate the effectiveness and efficacy of the organisations fatigue risk management strategies, policy & procedure. Case Study Background Information It has been reported that the same driver from question 6, was involved in a minor incident. When the driver returned to the depot, he opened the garage open via remote and reversed the truck into the bay. Unfortunately, whilst the driver was reversing, he didn't notice that the garage door dint fully open before he reversed and has torn the door from its tracks. The garage door now needs to be replaced. The driver had completed a Fatigue Risk Assessment Guide, just a week prior to this incident.
