The CEO wants a companywide IS (currently there are only two applications in the company: accounting and inventory), and appoints the VP of manufacturing, Mr. Ives, as the new CIO. Mr. Ives and his team derive the information requirements by examining the positions on the organisation chart and the job descriptions of the managers filling these positions. They then design all the system inputs, followed by the outputs. Mr. Ives then buys the computer hardware, contacts a software company and purchases an application software package from that company.

When the new system is being installed, transfers to new jobs are made and new forms and procedures are revealed. Users grow confused and angry. Within a week, conditions become chaotic..
