Of the 45 retrospective charts that were audited, 35.6% (n=16) were female, and 64.4% (n=29) were male. The age range of the patients from the chart review was between ages 19 and 63. Based on the chart review, the average age of the 45 patients was 40 years old; the average female age was 42.5 years while the average male age was 38.37 years.

Of the 45 charts audit completed post-implementation, 42.22% (n=19) were female, and 57.78 (n=26) were male. The age range of the patients from the chart review was between the ages of 18 and 72. Based on the chart review, the average age of the 45 patients was 39.13 years old; the average female age was 41.95 years, while the average male age was 37.07 years.

Based on the post implementation chart audit, 26 patients (57.8%) reported symptoms associated with depression, 27 patients (60%) had a depression diagnosis, while 21 patients (46.67%) were receiving treatment of some form for depression.

Of the 45 charts that were audited, 38 charts (84.4%) had a completed PHQ-2, and 29 (76.3%) of the 38 charts with a positive PHQ-2 had a completed PHQ-9.

7 of the 45 charts audited had no PHQ-2, however, 2 of the 7 charts had a completed PHQ-9 for a total of 31 charts having a completed PHQ-9. The PHQ-9 score on the 2 audited charts was less than five. There were 23 charts with a PHQ-2 greater than or equal to three. All 23 charts had a completed PHQ-9.

Of the 31 charts that had a PHQ-9, only 4 charts had a PHQ-9 score that was less than five, 7 charts had a PHQ-9 score that was five or greater, while 8 charts had scores of 10 or greater, seven charts had a PHQ-9 score of 15 or more, and 5 charts showed a PHQ-9 score of 20 or greater.

Of the 45 charts that were audited, there were 5 cases of severe depression; 7 cases of moderately severe depression, 8 cases of moderate depression; 7 cases of mild depression and 4 cases of no depression. Of the 27 patients who were diagnosed with some severity of depression, 21 patients were prescribed medication, psychotherapy, or referred to be seen by a psychiatrist. The other patients refused treatment at this time.

Even though 57.78% of patient records from the chart review indicated a diagnosis of depression, some patients refused to be treated as they did not feel that their symptoms called for psychotherapy or pharmacotherapy.
