1) Joe, a 23-year-old college student, was convicted of illegal possession of a controlled substance and is now serving a two-year probation sentence. He is seeing a college counselor. Joe's probation officer contacts the counselor and requests a report. Although Joe has not signed a release of information, the counselor complies, fearing that not doing so might cause the probation officer to file a negative report on Joe with the court.

2) Before seeing a family for counseling, a professional counselor gives the parents a written document explaining the process of counseling. After reading the informed-consent document, the parents sign it and bring the family to counseling. The informed-consent document is not given to or described to the children.

3) A family counselor in a mental health clinic has been having weekly sessions with two parents and their adolescent child, a juvenile offender who is about to be released from a detention center. In the past, the juvenile was known to be extremely dangerous and violent and had made generalized statements of hostility but with no intended or identifiable victim. The counselor took no action to block his release.

4) A residence-hall director who is responsible for hall discipline at a university dormitory has a master's degree in counseling. He is approached by students living in his hall who say they would like to address some personal concerns and want him to be their counselor. The residence-hall director arranges weekly sessions for counseling with the students.
