a. Patients are identified by their SSN, and we also store their names and age.
b. Each patient has one or several phone numbers stored in the database.
c. Some patients in this clinic may not have any insurance.
d. Some patients have one or two insurance policy (identified by Insurance lD). Each insurance policy has a start date. The insurance may be associated with several patients (such as for spouses, children).
e. Doctors are identified by their SSN. and we also store their names and specialty.
f. Each patient has one primary care doctor. and we want to know the start date that the patient has been with her primary care Doctor. g. Each doctor has at least one patient. Specify key attributes of each entity type and specify all the constraints (mandatory or optional participation. I-to-many, I-to-I, many-to-many) on each relationship type.
