While undoubtedly, not everyone has had the opportunity to be involved in or impacted by an Enterprise System initiative, we've all been involved in some form of process improvement effort, some Im sure with successful outcomes. Here, briefly describe a real-life experience with a process improvement initiative, ideally Enterprise System related (this is not required) that was not-so-successful.

What was the motivation and justification for the effort? How was the improvement identified and how was success intended to be measured? Was there a formal process improvement methodology followed? What was one thing you think contributed to the less-than-ideal results?
