You are a community visitor and you are responsible for visiting residential homes that care for people with disabilities. You have recently met a new resident called Sara who is living in an aged care facility.
Sara is 23 years old; she lives in a home with 4 other people who are much older than she is. They are also quite unwell and the home doesn't provide many activities or outings as the existing residents are bed bound.
Sara has cerebral palsy and is wheelchair bound. Her parents could no longer cope with her needs as they are ageing and recently her father had a stroke. Sara used to go out quite a bit and had a group of friends that she saw regularly. She also likes the movies and going out for lunch at the RSL with her parents.
Her parents tried to get her into a residential home near where they lived which had three (3) other young people living in it but Sara was refused access: they were told that her medical needs were too high and the residential home was not funded to care for someone with Sara's needs.
You have been visiting Sara and over the three months you have noticed that Sara is becoming withdrawn and is spending longer and longer in her bed. You contact her parents who are very distressed but as her father has deteriorated and her mother is unable to focus on Sara at this time.
8.a) What style of communication would you use when completing Sara's evaluation and assisting Sara to Identify and voice her needs, interests and requirements?
b) Describe how you will identify and implement adjustments required from her evaluation.
