You are a health information professional closely involved with risk management at General Hospital, a teaching institution. 3. A general description of what is Risk Management and an explanation about the growth and development of risk management in general and as applied to General Hospital.
4. Patient record requirements must address:
   1. proper documentation, using examples of a properly documented health record and a poorly documented health record;
   2. security concerns, including the active management of the availability of health records; and
   3. confidentiality, focusing in particular on the risk of talking about patients in appropriate spots, such as hospital elevators
5. Incident reports
   1. what they are,
   2. why they are necessary,
   3. how to and how not to complete one,
   4. how they are used, and
   5. how to protect them from discovery.
6. An explanation of the attorney-client privilege in the context of incident reports.
