You are a supervisor of new residential homes and renovation work. Recently an accident occurred on one of your renovation jobs where a brick veneer wall collapsed, injuring two labourers that had to be taken to hospital. There were two carpenters on site at the time, but they were at another part of the building. During the investigation, Safe Work NSW have identified that a labourer had cut the wall ties from the frame and dropped the wall instead removing the bricks progressively by hand as was detailed in the SWMS. Further investigations also revealed that the labourer said he had not seen the SWMS and needed the brickwork down, so it was easier to get the sheets of plasterboard lining that was being stripped off the remainder of the internal walls, out to the bins.
Using the table below list the factors that have contributed to the accident and what you, as the PCBU should have done to ensure these were accounted for or controlled.
