Case Note 1: Winnie was at the hospital for tests on 3/17/16. Those tests were done to determine whether she has a tumor on her thyroid gland. Homemaker assistance has been arranged. (The date for the tests is wrong. It should read 2/13/16. Correct the note accordingly in the space provided.) 

Case Note 2: Carmela is attending the partial hospitalization program 3 days a week. Today she appeared brighter and more talkative. Our interview focused on her need to find a more independent living arrangement. Partial hospitalization staff will assist her in looking at supported living programs in the community. (Carmela is actually attending the partial hospitalization program 5 days a week. Correct the note accordingly in the space provided.)
