Assessment Task 2: Workplace Project INSTRUCTIONS: If you are doing this course as a workplace-based student, you may be able to use your workplace's own documents, templates, and other resources. Make sure you have your supervisor's permission to use workplace documents and resources before you start this task. For this task, you are required to assess one client with complex needs to determine the client's developmental status and arrange for appropriate referrals. You will be required to question the client and their family/carer and document information relevant to the developmental status of the client. You must ensure that you will use a structured approach to the assessment and referral and follow relevant organisational policies and procedures. You supervisor must be present throughout the assessment, and a supervisor report is included with this assessment for your supervisor to complete during the developmental assessment. Confidentiality of the client is to be maintained at all times. The client is to be advised that the assessment and referral process will be used for assessment requirements however no names or personal information will be divulged. You are required to conduct your developmental assessment using appropriate assessment tools (these may be provided to you by your assessor or alternatively, you may have access to workplace documents/templates) You must prepare a report to address all of the required details below. Submit your report along with supporting documentation to your assessor. 1. Prepare for the assessment. Discuss the requirements of this assessment task with your supervisor. Ask you supervisor to assist you with finding a client with complex needs who you will be able to assess in the workplace. Complex needs may include: Mental illness Alcohol and other drugs Domestic abuse Poverty Disability Behaviours of concern Age Aboriginal heritage Cultural diversity. Plan assessment methods and gather appropriate assessment tools. Organise the details of the assessment with the client and their family as well as your supervisor. Ensure that all parties are aware of the date, time and venue for the conversation and assessment. Review any available documentation about the client's developmental status prior to conducting the assessment
