Sprinkle systems and fire extinguishers installed and regularly inspected and maintained. Fire evacuation and emergency procedures available. Chemical storage: Chemical containers appropriately labelled. Chemicals stored in approved containers (not drink or food containers) and away from food preparation areas. Workers trained to use chemicals safely (wear gloves, ) Electrical Equipment: Faulty electrical equipment removed immediately from service. Safety switches installed to guard against electric shock. Safety switches tested on a regular basis by a competent person. Licensed electrical contractor hired to install and repair electrical equipment. Electrical equipment regularly inspected, tested and maintained by a competent person. Workers trained in working safely with electrical equipment. Machinery and Equipment: Sharp edges and moving parts appropriately guarded. The equipment have appropriate safety instructions and signs on display. Workers strained to use machinery and equipment safely. 2. Hazards and risks arising from the analysis of the WHS Incident Data Base on the Key WHS Statistic Australia 2023 Serious claims 2020-21. The three main types of incidents most likely to occur in my workplace. - Falls, trips and slips of a person could lead to sprains and strains, back injuries, broken bones (traumatic joint/ ligament and muscle/ tendon injury, musculoskeletal and connective tissue diseases, wounds, lacerations, amputations and internal organ damage, fractures, nervous system and sense organ diseases, intracranial injuries) - Hitting objects with a part of the body could lead to fractures and dislocations, bruises and contusions, cuts and abrasions (traumatic joint/ ligament and muscle/ tendon injury, musculoskeletal and connective tissue diseases, wounds, lacerations, amputations and internal organ damage, fractures, nervous system and sense organ diseases, intracranial injuries)
- Heat, electricity and other environmental factors could lead to burns and electric shock (Wounds, lacerations, amputations and internal organ damage, burn)

Issues from team training day - Uncertainty with participating with the fire drill - Jeff Mackavilles injury during the fire drill - Uncertainty about why the water on the floor had not been cleaned up - Staff did not use mops and buckets for such purpose - The Wet Floor signs werent placed around the spill area
4. Hazard/s identified in the Hazard Incident Report Jeff sprained his left wrist. The water spilled on the floor led to slips and falls. To avoid this incident, if anyone in the workplace see the water is spilled, immediately use mops and buckets which are available in the cleaners cupboard, report to Stuart Percel and place the Wet Floor
