Complete a task analysis of TAKING OUT A SPARE PUMP FPR MAINTENANCE

The task is to take out a spare pump for maintenance. Assume that the spare pump is in parallel with the other pump and has a double block on each side of the spare pump and a bleed to isolate the pump. There is also a flowmeter, a thermocouple and a pressure transmitter in the line from both pumps.

1- Write a procedure for the operation.

2- Perform a Hierarchical Task Analysis (HTA) This should be accompanied by a schematic drawing of the plant which shows and labels all the items of equipment referred to in your analysis. The HTA should be performed on the operation the schematic drawing should be based on symbols, not actual pictures/images, and the drawing. The HTA should start at the point that the spare pump has been stopped after being in operation.

3- Based on the HTA, identify 5 safety critical steps and perform a Human Failure Analysis of one step marking the impact of at least 5 performance influencing factors.
