  • In the 12-18-month room, a child with a fever of 101 degrees was not sent home within a two-hour timeframe.
  • In the infant room, there was a case of hand, foot, and mouth disease, but there was no information posted for parents to review to understand the symptoms.
  • In the two-year old room, it was observed that the staff member changing diapers did not change gloves in between diaper changes.
