Gather a portfolio of evidence, countersigned feedback notes and develop a diary or logbook to show that you have been able to manage a team's effectiveness including the development of a team performance plan. Include the performance outcomes and measurements which were developed and communication strategies.

Provide documented evidence of actions or strategies to support team cohesion including two way communications, giving feedback, modifying policies and developing processes to resolve any workplace performance problems.
