Else if the region is east

Else if the region is central

The premium will be the insured value times $\mathrm{.}0010$

Else the only region left is midwest

The premium is the insured value times $\mathrm{.}0015$

Else the only other location left is rural

Need to repeat the above code with premium factors from the bottom row of the table
