The purpose of the change proposal is to provide staff with adequate training on determinants of healthcare in order to properly assess patient that come into the emergency department in order to service their needs better and efficiently.

Analysis of the need for change (why)

Staff appears to lack the proper usage of social determinants of healthcare deficits when treating patients. When approximately 3 out of 4=75% patients entered the ER during a single shift and appeared to be unkept due to their appearances staff assumed they were homeless. Because the individuals were unclean and carrying several bags and presented an odor, they were labeled as homeless.
