I am an OHS manager, one of the staff fell off the tree and hurt himself real bad (not following the right process.procedure). And i am doing some investagaion.

I wasn't at the secne during the incident, however there are some witnessess (company staff).

A detailed incident report is needed.

What would be the limitations of this report?

Please give me some ideas and write in detail (with ref.).
