The Health Board Case Study The health board runs a number of hospitals in North Wales. They have previously stored information regarding the hospitals, staff, and patients on paper. The Trustees have decided it would improve efficiency to move to a database system, and they have asked you to carry out the design and implementation of the system. Initially the system is only going to be handling in-patients who will be on a ward. Data Requirements Hospitals There are 20 hospitals in North Wales. Each hospital needs to have a note kept of its name, postcode, main reception telephone number, and general enquiries email address. Each hospital in North Wales also has a unique numeric ID. Each hospital has a number of wards. Each ward has a unique ID, and name. Each ward also has a specialism, such as coronary care, or general surgery. The most wards in a single hospital is 25, with an average of 15 wards per hospital. Staff The heath board employs a number of staff. It keeps information including the staff member's name (forename and surname), current salary and current position within the hospital. The position will be one of doctor, nurse, consultant, or secretary. Each member of staff has a unique employee identifier. A member of staff only works at one hospital. It is not necessary to know which ward at the hospital the member of staff is working on. The maximum number of staff employed in a single hospital is 2000, with an average of 1300. There are some members of staff who need to be able to see staff contact details but should not be able to see salary information. Patients A patient is someone who is treated in a hospital. Details kept about the patient include their name, date of birth, and sex. Next of kin details including a name and telephone number are kept for each patient. A patient may have more than one next of kin. Patients are identified by their National Insurance number. When a patient is admitted to a ward a record is required of the date of admission. Free form notes can be written by members of staff as part of the admission record. The maximum number of admissions to a ward is 500, with an average of 200. Medication A number of medicines are available to be prescribed to patients. Each medicine has a unique NHS identifier, a name, and contra-indications (when a medicine should not be prescribed). There are 250 medicines licensed for use by the health board. A doctor prescribes medicines to patients using a prescription. On average a member of staff writes 500 prescriptions, with a maximum of 3000. When a medicine is prescribed the information recorded is the start date, dose, frequency of administration, and end date. A medicine is prescribed on average 5000 times, and a maximum of 100,000 times. There are 1.5 million prescriptions recorded. Patients may be prescribed a number of different medicines, but each medicine prescribed requires a separate prescription. For legal reasons, it is essential that prescription records are never removed after a drug has been prescribed. On average a patient receives 20 prescriptions, with a maximum of 600
