In the last Module, we discussed the importance of case management and how case management is linked to referral sources with other community support programs. In this Module, using the same client from Module 01, you are going to be researching actual local and national resources that can assist you client.

 

Step 1: Review the case of your client from Module 01.

 

Step 2: Develop a list of stressors that your client is currently facing and discuss specifically what type of service(s) is needed to address each listed stressor. Be sure to include examples of each stressor and service identified.

 

Step 3: [Internet Search] For each of the stressors faced by your client, identify three local (in your city or state) community-based resources that could assist your client. Discuss a synopsis about each local resource, its relevance to your client's situation, and provide the URL for each source.

 

Step 4: [Internet Search] For each of the stressors faced by your client, identify three national (for example, national organizations, hotlines) resources that could assist you client. Discuss a synopsis about each national resource, its relevance to your client's situation, and provide the URL for each source.

 

Step 5: Present a reflective paragraph detailing your thoughts about the benefits of providing your clients with these local and national resources.
