Question 2 - HYPOTHETICAL ILAC QUESTION (22 marks) Professor Mary Mackay was a reputable 36-year-old Australian Academic who specialised in Geology. Professor Mackay, was a chronic asthmatic, who suffered a severe asthma attack while attending an academic conference at the Gold Coast, Queensland. An ambulance was called, and the attending paramedic administered intravenous salbutamol (intravenous salbutamol is rarely used in severe acute asthma). The paramedic's clinical judgment to administer salbutamol over adrenaline was due to Professor Mackay at the time having both an elevated heart rate and high blood pressure. Salbutamol is a common drug used for heart rate and high blood pressure. Despite some signs of improvement during transfer to Gold Coast Hospital, Professor Mackay's heart rate suddenly dropped and her condition rapidly deteriorated. The paramedic assessed that she was in cardiac arrest (respiratory arrest) and administered adrenaline. Despite the administration of both salbutamol and adrenaline, Professor Mackay sustained severe, irreversible brain damage from the deprivation of oxygen prior to arriving at the hospital and remained in a vegetative state for the following 5 years before her death in February 2021. Medical professionals argue that if she had been given adrenaline in the first instance, Professor Mary would have survived. It was found that the decision not to administer adrenalin did contravene the guidance in the clinical practice manual.
