You are required to develop a form (name, sir name, and email address) that receives data from the user to store in the database (Create a database and tables in PhpMyadmin). You may use the existing controller and model or you can create the new one to insert the data from the user. Create a route to insert data and display data.

* Submit by capturing your DB, model, controller, and the result of the program (convert the file into pdf and attach your answer).
