Mr. Smythe refused to accept only the front page and requested that the Manager of the GWS health centre action the request urgently. GWS continues to refuse to grant access. GWS holds and controls all the medical records of patients who attend their health service centres. Dr. Feelgood is a general practitioner working at the GWS health service centre located in Adelaide. Mr. Smythe was feeling quite angry and confused one evening. His anger boiled over when his partner started to discuss his recent behaviour, After listening to his partner talk for a bit, Mr. Smythe became extremely irate and started to smash the dishes against the wall. His partner ran out of the kitchen, locked herself in the bathroom and called the police. Mr. Smythe was taken to a mental health facility at a hospital from 31 May 2019 to 1 June 2019. Upon his release, the hospital produced a discharge summary that indicated the dates of admission, some background to the hospitalisation, diagnosis, medications, treatment and reasons for discharge. At the end of July 2019, Mr. Smythe contacted the hospital and requested a copy of the discharge summary. The hospital required Mr. Smythe pay a fee to gain access to the discharge summary.
